Silverchain has partnered with COORDINARE South Eastern NSW Primary Health Network (PHN) to deliver a falls prevention program.
This is a free, eight week group exercise and education program available in various locations within South Eastern NSW.
Benefits to your patients
- Reduces the risk of falls by improving strength and balance through exercise.
- Sessions are held locally and in person, providing an opportunity for social interaction.
- Increases confidence and overall health and wellbeing.
Eligibility
This program is available to people who can mobilise independently in the community and are aged:
- 65 and over, or 50 and over for Aboriginal and Torres Strait Islanders.
- under 65 and have a health condition that impacts mobility such as Parkinsons disease or has an acquired brain injury.
What your patient will receive
An individual comprehensive assessment before commencing the exercise program to ensure they can safely participate and will benefit from the program, and to establish a baseline.
Basic equipment to use with the program, at no cost and for them to keep.
Eight, one-hour sessions consisting of:
- Exercises delivered by an accredited Exercise Physiologist (AEP).
Education covering topics such as:
- Falls statistics and most common injuries.
- How occupational therapists can help reduce falls.
- The importance of exercise and skin integrity.
- Incontinence and where to seek information and advice.
- An overview of assistive devices, and ambulance and falls pendants
- Seniors rights service information.
- A tailored exercise program to follow at home, with the option of virtual monitoring.
- A final assessment to measure changes.
Opening hours:
Program will Commence on the 12/01/26 for one on one assessments and group sessions starting on the 19/01/26 running up to the 16/03/26.
Sessions run for 2 hours from 10:30am-12:30pm.

50 Plus, 65 Plus
Participants who would not be suitable for the program:
Significant cognitive decline or dementia.

Beginner
