You can now access the latest falls prevention and healthy ageing information on the NSW Health Active and Healthy website. It brings together free information in one place, including printable resources and online tools. See below for further details.

Current Healthy Ageing resources
- Falls prevention fact sheet - simple tips to help reduce your risk of falls.
- My plan if I should fall worksheet - a worksheet to help you plan what to do and who to contact after a fall.
- Physical activity manual and logbook - simple strength and balance exercises you can do from home.
- Available in Arabic, Simplified Chinese, Traditional Chinese, Greek and Italian.
- Online Healthy Ageing learning modules - practical information on healthy ageing, falls prevention and home safety.
- Home Safety Checklist - identify common falls hazards and make your home safer.
- If you have a fall - a visual guide on what to do after a fall and how to get up safely, where possible.
Community members who require printed resources can request copies of the Falls Prevention factsheet and Physical Activity Manual and Logbook through the Centre for Population Health. For enquiries, please contact MOH-ActiveAgeing@health.nsw.gov.au
More ways to stay active and reduce your risk of falls
- Active and Healthy exercise directory - Find local physical activity and falls prevention programs near you.
- Get Healthy Service - free phone and online support from a trained health coach. Participants can receive up to 10 coaching calls to work on goals such as improving balance, strength and flexibility.
Clinician resources
Clinicians can access a range of evidence-based falls prevention and frailty resources to support assessment, education, and referral pathways in both hospital and community settings. Resources include:
- Clinical Excellence Commission post fall management resources
- Agency for Clinical Innovation Frailty resources
- GP and practice nurse resources and Falls prevention clinician education programs, from the NSW Falls Prevention and Healthy Ageing Network
